Patient self-report: An elderly woman explains that over the past several weeks she has been feeling fatigued more than usual. Even though she hasn't been eating much she often experiences bouts of pain in her stomach. She decided to make an appointment to be seen when her stool was light in color, yet her urine was darker.

Symptoms: When first examining her you notice the unmistakably strong order of cigarette smoke. When you ask her if she is a smoker, she mentions that she has been smoking since she was a young woman. "That's what all the women did back then", she said. She begins to scratch her skin seemingly nervous but when you ask her about the itching, she admits that she has been scratching more than usual too. Initially you consider that a healthier more active lifestyle with relaxing, smokeless walks along the beach is all that is needed but something bothers you about the yellowish tint to her skin.
