Question

Mr. WC is an 82-year-old male with PMH of uncontrolled type 2 diabetes, hypertension, and hyperlipidemia. He was recently brought to the hospital after being found by his neighbor in his yard with an altered mental status and multiple wounds. It was discovered that his blood sugar was 1200 mg/dL at admission, and he has multiple wounds on his toes and feet that have been left untreated. He also was extremely dehydrated. Mr. WC does live alone and has recently had difficulty getting to his doctors' appointments since his wife's passing, so unfortunately, he has been without his medications for an extended amount of time. You review Mr. WC's chart but unable to determine his preadmission weight. At admission, he weighed 225 lbs. Today, he weighs 220 lbs and is 5'11, making his BMI 27.9. Mr. WC has been in the hospital for over a week due to wound care protocol. He has had a poor appetite during this time; often, only consuming <50% of his needs. He does not like oral nutrition supplements and adamantly refuses to drink them. After discussion with the doctor on the case, it is decided enteral nutrition is the next step for Mr. WC.
