The HIM Manager at the critical access hospital has a problem with the two doctors on staff that involves transcription. The HIM department transcribes the hospital and doctors' clinic notes. One of the two doctors made it very clear that he has "generic dictation" for clinic visits- all he may say is "regular female checkup over age 60 - no abnormal findings". He expects the transcriptionists all information from a "normal" template that he has provided.
Templates provide generic information for the transcriptionist to use to complete the report. The reports for all the patients that use the same template would be the same except for identifying information.
Is this process acceptable or not? Provide a rationale for your answer. If it is not acceptable, describe what the HIM Manager should say to the doctor. These two doctors make up the whole medical staff. When they don't get their way, they threaten to leave. The hospital went to a lot of effort and expense to recruit the physicians.
