Question

D.E. is a 72-year-old female admitted to the medical unit by Dr. Santos with a diagnosis of Clostridium difficile after being treated with antibiotics for pneumonia. D.E. is a DNR/DNI with no known drug allergies (NKDA). Responding to a bed alarm, the nurse enters D.E.'s room and finds D.E lying on her right side on the floor by the bedside commode. D.E. is alert and oriented to person, place, time, and situation. She states she fell when attempting to get on the bedside commode. She denies hitting her head but complains of pain in her right hip. The nurse obtains a set of vital signs: Temp: 36.7C (98.06 F), Heart Rate: 102, Respiratory Rate: 22, Blood Pressure: 118/74. The nurse and the unlicensed personnel attempt to stand the client and place her in the bed. The client is unable to bear weight on her right hip. The nurse notes ecchymosis and edema to the right hip. After lowering D.E. to the floor, the staff use a lift device to get D.E. in her bed. Once D.E. is settled in bed with the bed alarm activated and the call bell in reach. The nurse calls the healthcare provider.
