Question

Resident came from Mercy Hospital after being treated for subdural Hematoma that was due to the resident fall, resident did not loose conscious when he fell, was sustained a c6 and T2 fractures, SAH, SDH, AND facial fracture. Resident wear soft collar. Resident has medical history of chronic atrial fibrillation, primary hypertension, acquired hypothyroidism, metastatic melanoma, Chronic kidney disease stage 3, GFR 30-59ML/MIN (HCC), SUBDIRAL hemmorrhage following injury, no loss of consciousness, fhx smoking. Resident is admitted to us today. Resident arrived around 1330, came via facility car with the driver and walked into his room on Noth, room number North 12 A. Resident is alert and oriented time 4 (to person, time, date, situation). Resident reported was happy to room with his wife and he is excited to be here for PT, OT. Denies pain, dress appropriately for weather. This nurse educate the resident how to use is call light, oriente him to his room, and routine (meal, shower, etc..) Resident sussessfully demontrate understanding. put this in good english flow.
