Mr. Singh identifies as male and was admitted yesterday due to ischemic stroke confirmed by CT angiogram. He was not a candidate for either thrombolytics or endovascular therapy. His current deficits include right-sided flaccid hemiplegia, dysphagia, and mild expressive aphasia. He is currently NPO, awaiting assessment by speech language pathology (SLP). He has end-stage kidney disease (ESKD). He is anuric and on hemodialysis for which he has an AV fistula in his left arm. He also has insulin dependent DM2, CAD, and biventricular HF. He had an aortic valve replacement 8 years ago for aortic stenosis. Admission VS: BP 164/80, HR 74, T 36.8° C RR 16, O2 sats 98% on RA.
