The improvement of this interaction can be done by synthesizing the data in one column. When a patient arrives on the unit and all the admission questions are done, the checklist for the questionnaire goes away, yet each department has other mandatory data that needs to be collected. Most of the time this information is not gathered because it is sort of hidden in the EHR and we must go looking for it in a section called navigators. My department requires us to complete elopement and suicide screen, most of the time I forget that because it is not highlighted in the admission section. Both elopement and suicide screening are important, my hope is that they can be placed together with other questions or even be highlighted for easy nursing access. Another recommendation is to condense the pain documentation, it is such double documentation in my unit. When on the MAR, you document the pain assessment when giving the pain medication then you go back on the flow sheet and enter the information. This takes so much time that nurses barely have. Also educating staff on proper documentation is some ways to improve effective flow of care.
