Question

A 23-year-old client is seen in the clinic for a follow-up after a surgery for a fracture of the right arm. Nurse's Notes Vital Signs Provider's Orders 4/22 1000 A review of notes shows that this client had an open reduction and internal fixation of the right humeral fracture. Surgical site has been immobilized with a cast. Last documented assessment findings are unremarkable and within expected limits. Skin above and below the cast is intact; skin is warm to the touch. Hand and fingers are slightly swollen. Client reports itching on the surrounding skin above the cast, and becomes intense when sweating. Denies pain in the affected area. Reports sensation to the hand and fingers; hand and finger mobility are present. Radial pulse present and strong bilateral. Capillary refill is less than 3 seconds. Cast is dry. Health care provider has been notified of the findings. Client was advised to elevate the affected site at the shoulder level to reduce swelling. 4/26 1200 Client reports increasing pain in the area covered by the cast stating, "A mild discomfort and itching started a day ago, but the pain increased when I woke up this morning." Pain is rated at 4/10. Client.
