Functional Assessment Assignment

For this assignment you will be using the three assessment tools below on someone over the age of 65, or someone you believe has a "disability" of some kind, in order to assess their functional status. Katz Activities of Daily Living

1. Lawton Instrumental Activities of Daily Living Scale

2. Geriatric Depression Scale (Short Form)

Step 1: Interview your "client" and gather information regarding their brief health history (age, gender, social history (marital status, occupation, smoking/drinking/drugs), current medications - name only, and brief past medical history; NO review of systems).

Step 2: For each assessment tool:

1. Write a short paragraph summary of what the tool assesses.

2. What were your "client's" results? (Do not include the actual forms).

Step 3: Final paragraph: Write an overall summary of your "client's" functional status based on the results of the three survey tools.
