Mrs. Cooper, 87 years old, is nearing being discharged from an acute-care hospital after she suffered a hip fracture resulting from a fall. She uses a walker, and her Timed Up and Go score is 28 seconds. Mrs. Cooper has moderate-to-advanced-stage Alzheimer's disease, but up until the fall, she could still perform all basic activities of daily living. Her husband, Mr. Cooper, who is 90 years old, has been her primary caregiver and has managed many of the instrumental activities of daily living. They have lived in their two-story home for 53 years. Their child, Mr./Ms. Allen, 65 years old, lives nearby and helps out with shopping and laundry on the weekends. Mr. Cooper is in good health for his age, although he is unable to assist Mrs. Cooper. His brother, 92 years old, lives in a nursing home, and Mr. Cooper has vowed he "will never go down that road."
