A 19-year-old man with long-standing attention-deficit/hyperactivity disorder presents to the psychiatric clinic on his college campus after being referred by the resident assistant in his dorm due to concerns that he has been displaying depressive symptoms. Although he states that he is enjoying the college experience, he reports significant struggles with academics since starting his freshman year about 3 months ago. About 2 months ago, his primary care provider agreed to increase his atomoxetine from 40 mg to 60 mg per day in order to help him with concentration and organization. He reports that the increase did not help with his attention or grades, and that he has become increasingly sad and distressed over the last few weeks about flunking out of college. In addition, he reports trouble getting to sleep during this time and reports self-medicating his depressive symptoms with excessive alcohol use over the weekends and with daily cannabis use. He points out that he lives in a US state that has legalized marijuana for anyone over the age of 18 years and that both substances help his mood. The campus provider assesses that the student does not currently require specific substance use services (e.g., outpatient, residential, inpatient)
