CASE SCENARIO

You are a practice nurse working in a small general practice within a large indigenous community. There is a high level of diabetes and in collaboration with the GP, you plan to provide diabetes education. You contact diabetes organization and set up a program to provide education and support. The GP identifies patients and provides the facility for education, while your role is to conduct education sessions and monitor self-management.
