A client has T2DM. You have been asked to perform and record a chemical reagent strip urinalysis, colour blindness test, and blood glucose measurement. The client complains of excessive thirst and has asked for water immediately

The results of the assessments show that the client's urinalysis: a glucose level of 50; blood glucose level of 19.1 and no abnormal findings in the colour blindness test. Record the results in the client's progress notes. Ensure you adhere to the clinical guidelines, organisation policies and procedures and confidentiality requirements, and include the date, time the test was performed with your signature. Please use relevant medical terminology, and ensure you record the side effects that your patient had mentioned, and what you did (or would do) in that situation.
