Question

A baseline coding audit of debridement services performed in a hospital was conducted on 25 claims. The review indicated that in roughly half the medical records reviewed, the documentation did not support the code assignment. The author believes that coders may not fully understand the coding guidelines for coding excisional verses non-excisional debridement. She also believes that the physicians are not informed on the documentation requirements necessary for coding debridement services.
