The patient is a 59-year-old male who has been sent to the hospital by his family physician due to severe heartburn, vomiting, and dehydration. The patient is admitted by Dr. Michael Toast, a gastroenterologist, who scheduled several tests, including an EGD which he performed. All the results indicated that the patient has not only severe GERD that is causing a small amount of scar tissue to develop in esophagus the but also a sizable hiatal hernia, which is the underlying problem. The patient has been treated for dehydration with continuous IV fluids, for the vomiting. The patient is also taking Carafate to treat the severe GERD. After consulting with a surgeon, Dr. Toast recommends to the patient that must be repaired to relieve all his symptoms. The patient agrees and has a Nissen fundoplication performed by the surgeon, Dr. Raymond George. The surgery is successful, and within a few days the patient is eating small amounts of solid food without any difficulty. The patient is discharged home on day 11 with home health care to begin on day one of being home for his hospital condition and shall continue for approximately 4 weeks.
