A 68-year-old male client presents with complaints of a progressive onset of exertional dyspnea that has occurred for years but has worsened in the last few months. He reports an occasional cough, congestion in the morning, and chest pressure with activity. Three days ago, he was exercising on the treadmill when he experienced brief lightheadedness and passed out for about 20 seconds. His son was able to arouse him quickly. Assessment showed clear breath sounds in all lung fields, normal heart sounds with a 3/6 systolic murmur heard at the second intercostal space right of the sternal border.
