Jane Smith Jane is an 82-year-old female resident living in the Rosedale aged care facility. Jane has an extensive medical history including diabetes, high cholesterol, osteoarthritis, colon cancer and chronic renal failure. Jane was diagnosed with Multiple sclerosis last year. She was transferred to Rosedale aged care facility last year. Jane is a high nursing care resident. Jane requires full nursing assistance in terms of her activities of daily living. Jane requires hoist transfers from bed to chair. Jane is on a thickened pureed diet as per dietician instructions and dietary requirements are clearly mentioned on Jane's care plan. Jane is fully dependent on the carer for her nutritional needs. She is required to be fed all three times during the day. Her daughter comes and assists her with the midmorning and evening snack time with feeding as well. Jane has extensive dysphagia as per her medical condition. She needs to be sitting upright at 90 degrees while feeding to avoid aspiration and risk of choking. Jane care plan has all the details about her feeding regime.
