Question

A sonogram showed no gestational sac, but there was a small mass within the uterus that could represent a gestational sac. A repeat hCG titer done as 15,000. A second hCG titer came back as 18,000. On admission, her hemoglobin was 12.9, dropping to 8.1 in the afternoon, and at 7.9 the following morning. Based on these findings and the severe abdominal pain (9/10), the diagnosis of a possible ruptured ectopic pregnancy was sufficient to proceed. The patient was taken to surgery and a laparotomy was performed with findings of a right ruptured ectopic pregnancy. The right salpingectomy was performed with no complications. There was no fever reported. The abdomen was soft. She had a normal bowel movement. FINAL DIAGNOSES: 1. Right ruptured ectopic pregnancy with hemoperitoneum.
