Assignment:

Using the following scenario, make a PICOT question.

In my short time practicing as a nurse and while in nursing school I've seen this happen, but I have not really found if it's an evidence-based practice. Senior nurses told us that anytime a patient was hypotensive, regardless of the circumstances, it was best to place them in the Trendelenburg position. Almost all nurses follow this approach, especially in the emergency room, where these patients typically arrive first. However, I've always questioned whether or not this nursing intervention was evidence-based. For this assignment, I will search for a journal that examines this intervention so that I may have a better understanding of its efficacy and validity as a frequently used nursing intervention.

Use of the Trendelenburg position as the resuscitation position: to T or not to T? by Natalie Bridges and Adrian A Jarquin-Valdivia and Why don't we use Trendelenburg? by Myrna Schnur are the research publications I have selected to analyze this intervention of placing hypotensive patients in the Trendelenburg position to restore normal blood pressure.

The purpose of this study was to conduct a literature review on the use of the Trendelenburg position as a resuscitation posture for hypotensive patients. The authors searched for literature on the posture utilized for resuscitation using PubMed online, referenced bibliographies, critical care textbooks, and Advanced Cardiac Life Support guidelines. Because the data were heterogeneous, only relevant papers and chapters were summarized. Regarding the specifics of this study's methodology, only limited information was provided.

There were eight peer-reviewed articles on the resuscitation perspective, as determined by the research. The review includes pertinent information from two critical care textbooks and the Advanced Cardiac Life Support guidelines. Literature pertaining to the subject was few, weak, and appeared to be led by "expert opinion." The authors found that the general "tendency" of the available data suggests that the Trendelenburg position is probably not an effective posture for resuscitating hypotensive individuals. They suggested that more clinical research is required to establish the ideal resuscitation posture.
