Question

In my surgical office the nurses are responsible for taking the initial history and physical information from the patient. These patients are pre-surgical patients and a crucial piece of information that we discuss is their history of previous surgeries and how they managed their pain. If they don't have a history of previous surgeries, we ask them what their expectations are and then explain our plan of action to control their discomfort. An important question to ask any patient is their use of any substance disorder, mental health conditions and any history of chronic pain. We also review the risks and benefits of opioid use, safe usage, and disposal of any leftover opioids. Concurrent medications are also reviewed and the patients is advised what to stop prior to surgery and when they can resume their prescriptions after surgery. We discuss a pain management plan, that includes adding Acetaminophen and NSAIDS at the appropriate time after surgery. Due to the Opioid crises in this country, prescribers have limited the quantity of pills they prescribe at a given time. The patient is informed of this at their pre-surgical visit. Prior to the pre-surgical visit, I do the NYS I-Stop.
