This assessment requires you to undertake research, develop, implement, evaluate and present your findings of an early intervention and/or health prevention and/or health promotion program to address the needs of a specific group/community.
You will be required to demonstrate the criterion identified in each of the three parts for this assessment.
Part 1 - Research mental health and wellbeing issues that are prevalent in individuals, groups and the community
• You are required to undertake research into the current mental health and wellbeing issues that are ofcommunity concern and document your findings in a written report. You will need to document the issues that are prevalent in individuals, those that affect groups and those that exist within the community.
Identify, access and review information sources on population health, demographic, social and epidemiology trends. You will be required to document the information you have reviewed in your written report including statistical information where relevant. You will also need to cite where you located the information such as who wrote or published it, how it was written and when it was written to ensure your sources are reliable, accurate and current.
Based on the research you have undertaken, you will choose one mental health issue that you will develop a program for. Using the following questions as a guide, determine key issues and objectives associated with the mental health issue and document these in your written report.
• What problems currently exist (the determinants)
• Who is being affected by these problems (males, females, youth, adults, cultural groups etc.)?
• What outcomes will the program focus on?
Part 2 - Develop and implement a plan
• To support the development of your program, research 5 programs and initiatives that have been implemented in Australia. Outline the details and features of these programs in your written report.
• Research current preventative health care perspectives and determine those that may be able to be integrated into your program, preventative measures may focus on the following:
• fitness, exercise and physical activity
• nutrition
• environmental health
• mental health
• sexual and reproductive health
• You will need to document the programs you have researched in your written report
• Based on the information you have gathered, you will need to document (using Microsoft word) the way your program will be structured to address the identified needs and objectives. The structure of your program will need to address the following information:
• Program description
• Issues/Determinants (what it will be addressing)
• Target group/s
• Specific goals/objectives
• Who will be involved (professionals, workers, stakeholders etc.)?
• What will be used to achieve this (tools, strategies, interventions)
• Where will this take place (communities, schools, sports clubs, health services)
• How will objectives be measured/evaluated
• Develop an action plan in table format using Microsoft Word. Your action plan should be the instructions of how to achieve the program plan/structure and will need to include:
• The activities that are required to achieve objectives
• Who will complete each activity?
• The resources to be used
• When the activity is expected to be completed
• Where you will undertake your activities
• Evaluation method to be used
• In your written report, document and outline how you implemented your program. This should include information relating to your action plan such as the activities you conducted, who was involved, what you used to achieve it, where your activities were carried out etc.
Part 3 - Evaluate program outcomes and present findings
• Include a summary of your findings in your written report (approx. 300 words) that outlines if your program was successful in meeting its identified objectives and where relevant what recommendations for change you would suggest. Your written report will also need to identify who you consulted with and the methods you used to evaluate your program.
