FACILITY PLANNING

Project: Pediatric Oncology - Opened a Pediatric Bone Marrow Transplant (BMT) program and a Pediatric Chemotherapy Certification Program (in collaboration with the adult team)

What went well?

  • Researched the requirements for Oncology Certification for Pediatrics and Adults
  • Completed a cost analysis to set up the certification process at the hospital compared to paying for staff to attend outside of the facility (it would be more cost effective to do in-house)
  • Assessed how many resources would be needed for training and how frequent to offer the classes (agreed upon 2X a year for adults and pediatric RN's) after the initial training of all staff.
  • Obtained feedback on the classes and if the staff needs were met (successful implementation of Chemotherapy certification).
  • Agreed upon requirements for certification within 6 months for new hires
  • Assessed location for BMT patients (would take a quad away from Pediatrics) and rebuild with 1 BMT unit with bathroom and 1 Positive Pressure room in the same quad). Usage would reduce from 4 rooms to 2 -but costs of BMT room use would make up costs.
  • Developed educational program for BMT nursing care for RN's.

Opportunities for Improvement?

  • Needed to clarify upon hire and to existing staff that certification is a requirement for employment.
  • BMT usage in the Adult and Pediatric units were less than budgeted impacting financial targets.
  • Volume impacted ongoing staff competencies (set for annually) but there were not enough patients for sufficient staff numbers to maintain competency.

In the end, our projections from Accounting and Marketing on the volume analysis was not accurate. The majority of patients (even those at our facility) opted to have their BMT procedure at the local Children's Hospital due to their volume/experience or insurance requirements.

Five years was donated for start up, if volume did not increase and outcomes equal targets, the BMT program would be closed at that time.
