Question

Daisy has had great difficulty mobilizing- related to ongoing pain with weightbearing to her repaired fracture and the persistent tremors, muscle cramping, altered gate and stooped posture. She also may have aspirated related to her ability to manage oral liquids (PD impacts, deconditioning and some concerns of reflux). She has developed a gram-positive streptococcus pneumonia and experienced acute progressive respiratory failure. She was placed on non-invasive positive pressure ventilator support, a PICC line and enteral feeding tube (oral gastric) were placed. After a course of ventilation support, Daisy has successfully weaned to oxygen via nasal prongs in the past 36 hrs. She has just been transferred to the med-surg unit for continued rehabilitation and recovery.

Daisy's wife is particularly unsettled by the notion Daisy will need supportive care at home. How will you explore these concerns, provide support, and facilitate solution building in preparation for transition to home.
