Question

Mrs Greenfield has chronic pain. She is on regular oral pain medication and a pain medication patch. She reports every day that she is still in pain. The care staff report this to the nursing staff. The GP has not reviewed Mrs Greenfield's pain medication in 6 months. There has not been a reassessment of her pain and pain levels for several months. Her pain management care plan has not been reviewed or updated in the past 3 months. As an EN what actions would you take and why/rationale?
