Question

Reducing readmission in heart failure patients through weekly targeted education in the digital patient portal Students will present a proposal designed to engage patients/customers and improve clinical outcomes and/or processes, meet external standards/requirements, or improve patient/customer satisfaction with the organization. Identifies practice gap and potential implications Consider the social determinants of health (SDOH). SDOH are one of three priority areas for Healthy People 2030, along with health equity and health literacy. Five key areas of SDOH include: healthcare access and quality, education access and quality, social and community context, economic stability, and neighborhood and built environment. Some examples of SDOH included in Healthy People 2030 are safe housing, transportation, and neighborhoods; polluted air and water; and access to nutritious foods and physical health opportunities.
