Recognize Cues What matters most? Identify important information from the patient assessment and requires follow-up? Abdominal Distention and Bowel Obstruction: These are immediate concerns as they might lead to further complications due to my patient lacking bowel sounds. I am going to assess bowel sounds and abdominal rigidity or tenderness. May need surgery. Will require an Xray to see obstruction. Hypoxia: Could cause further respiratory complications- Patient was ordered oxygen and has bi-pap nearby. Keep monitoring SO2 and his breathing- Repositioning. Hypertension: Monitor for low blood pressure an pulse were distant.- check with doctor if blood pressure medication is too low and ask to with hold meds. Bradycardia (51 Bpm): Requires monitoring given the normal history of hypertension and now hypokalemia. Hyperglycemia: Monitoring his diabetes is vital- administer or not administer Chronic Diarrhea causes side effects of electrolyte deficiency- monitor. Potassium administration: being given K therefore watch for hyperkalemia. Chronic Immobility: Patient must be turned every hour. Watch for contractures Patient Edema: Patient was given Lasix and will be voiding a lot / Watch for dehydration. Patient order is no food. He may have water and apple juice when he takes his medication.
