Comprehensive nursing assessment completed with regular chest rise and fall, no s/s of infection noted. 2000 Medication administered (See MAR). 2100-2300 Patient transferred to his bed via Hoyer lift, continuous oxygen reconnected. Percussion vest therapy was initiated, Patient tolerated it well. Tracheostomy care performed using SN, s/s of infection noted, patient tolerated it well, with no s/s of respiratory distress. Tracheostomy site remains without any redness, drainage, swelling, or infections noted. Sputum is pale yellow with no odor present. 0000-0200 promotes 55ml feeding initiated 55ml/hr. via G- tube, patient tolerated it well. 0300-0500 patient lying in bed eyes open, respirations even and unlabored. 0600 emptied suction canister, washed syringes, Patient is lying in bed eyes open, and report given to the incoming nurse.
