Any information that can identify a patient-like their name, ID number, or date of birth-is protected by a law called PHI (Protected Health Information). Other private details, like the person's medical history, past drug use, diabetes, medications, and treatment plan, are also protected. This also includes notes from therapy, what kind of therapy they had, how they're doing, and which doctors or therapists are helping them. Even the instructions given when a patient leaves the hospital (discharge instructions) are protected if they include personal health information. Ethical: It's important to make sure the patient understands everything in their discharge plan and agrees to it. They should sign it to show they understand - this is missing from the discharge plan. It is important to discuss and document on the discharge summary what supports the patient will have once he is home. It is important that the patient not be alone during this time of transition - this is missing from the discharge plan. A good and fair discharge plan should also have the name of the doctor, their phone number, and the date and time of the patient's next appointment - this discharge plan only says follow up with PCP.
