Question

You will be provided with a copy of a care plan along with a profile for a person with a disability with complex needs.

You will then undertake a consultation with the person in a simulated work environment to discuss the plan. The person will indicate at least two (2) areas where the plan is insufficient.

In consultation with the person, and in line with your role, organisation and/or program guidelines, make necessary revisions to the plan.

A signed observation by the assessor will need to be included in this activity as proof of completion.
