Question

A 30-year-old male patient visits the clinic for his cough which started 2 weeks ago and progressively worsened. The cough is non-productive. He has had intermittent, low-grade fevers and feels short of breath. He is an active smoker and consumes about half-pack of cigarettes a day. He denies any history of pulmonary diseases. The patient is very thin, coughs frequently and appears to be in moderate respiratory distress. Lung examination reveals decreased breath sounds and rhonchi in all fields. A chest x-ray shows a bilateral interstitial infiltrate with a "ground-glass" appearance. He discloses that he is HIV positive.

1. What is the most probable diagnosis and etiologic agent causing the infection?

2. What is the most probable risk factor which contributed to the development of disease? Discuss thoroughly.

3. Provide the treatment for this case of fungal infection. Since the patient is HIV positive, would you advise a prophylactic treatment? Why or why not?
