CC: Dorothy Porter, 29 year old female, presents with a rash on her hands and wrists. VS: T 98.8, P 72, R 12, BP 118/58, Ht 5'9" Wt 135. HPI: The rash started 1 week ago and she has never had this before. It appeared first on the right wrist as a few bumps, than spread to left wrist and fingers. It is very itchy and worse at night. She used an oatmeal bath that did not help. She has not put anything on it. She denies fever or chills; no recent cold symptoms. Her partner does not have it ("as far as I know"). She washes hands often but does not use harsh chemicals without wearing gloves. PMH: Chronic Illness: none denies CAD, DM, Cancer Meds: OCPs x 5 yrs - Lo Ovral Med allergy: aspirin (upsets stomach) LMP: 2 wks ago; regular, monthly; 5 days duration Hosp/Surg: none SH: occupation nursing assistant and lives with male partner x 4 yrs. No/never smoking, 1 - 3 glasses of wine on weekends, sexual active hetero, one partner.
