A urinalysis was suggestive of urinary tract infection and she was treated empirically with Macrobid (nitrofurantoin). You receive her urine culture results 2 days later as follows: E-coli >100,000 colonies Sensitivities Nitrofurantoin I Amoxicillin (Penicillin) R TMP-SMX (Sulfa-trimethoprim) S Cipro (Fluroquinolone) S Based on this information, appropriate response would be:

Stop Nitrofurantoin and begin Cipro (Fluoroquinolone) as it is safe for pediatrics and during pregnancy Continue Nitrofurantoin and check with the patient. There is only intermediate resistance to nitrofurantoin, so it should work well enough. Stop Nitrofurantoin and change to TMP-SMX (Bactrim DS, Sulfa) Continue Nitrofurantoin but add TMP-SMX (Bactrim DS, Sulfa) to make sure infection is treated.
