Client reports a lack of appetite in the last 1 to 2 weeks, and losing 8 lb. (3.6 kg) during this time. Client has two to three alcoholic drinks each day and has not been taking prescribed medications or preparing meals since spouse died 2 months ago. Client states last drink was about 12 hours ago. Client started to experience a burning feeling in both feet and went to see provider, who admitted client to the hospital for treatment and monitoring. Alert and oriented to person, place, time, and events. Lungs clear bilaterally. S1 and S2 heard on heart auscultation; no S3, murmurs, or rubs heard. Radial and dorsalis pedis pulses strong. Abdomen large, round, with active bowel sounds in all four quadrants. Skin pale, dry, cool to touch. Weak hand grasps bilaterally. Able to feel light touch in feet. Reduced ankle reflexes. Vital signs: temperature, 98.2°F (36.8°C); heart rate, 92 beats/min; respiratory rate, 18 breaths/min; blood pressure, 112/60 mm Hg. Oxygen saturation, 95% on room air.
