You and your co-worker, Evans, are at the med cart preparing your 1200 meds for your respective clients. You saw that Evans took out a cloudy insulin vial from a drawer on the med cart. He said that one of his clients was on insulin QID.

1. What nursing interventions should have already been done before preparing the insulin

You noticed that Evans drew up the cloudy insulin using a tuberculin (TB) syringe. From your knowledge of insulins and the time of day, you are thinking that Evans drew up the wrong drug using the wrong syringe. You told Evans that the insulin he is supposed to give now should not be cloudy and that he has to use an insulin syringe, not a TB syringe. He responded, "of course it is supposed to be cloudy, and this syringe is just fine."
