You are caring for a client on a medical-surgical unit. The client is postoperative after abdominal surgery and has patient-controlled analgesia (PCA) prescribed for postoperative pain. Two hours after surgery, the client calls for assistance. As you enter the room, you assess that the client is tearful, is guarding her stomach with her hands and arms, and states "I am in so much pain. I cannot get comfortable." When asking the client to rate their pain on a 0-10 pain scale, the client's response is "It feels like more than a 10!". The incisions are well approximated, and the staples are dry and intact. The abdomen is distended and tender to touch. The client has hypoactive bowel sounds in all four quadrants. The client has a sequential compression device (SCD) in place.
