Mr. RJB, a 55-year-old park ranger, visited his primary care provider because of a persistent headache and some neck stiffness. He found it increasingly difficult to walk, and his hands felt sore. Two weeks before his medical visit, he felt as though he had had the flu for a few days. He thought that his present symptoms were related to the flu. His temperature was 38.2 C. He had some optical photosensitivity to bright light. On further physical examination, some right-sided facial drooping was noted. Further questioning of the patient revealed that he had noticed an insect bite on his leg a month ago. The site had a red appearance, with increasing redness spreading out from the bite (Figure 1). The rash self-resolved with treatment.
