In another nurse's decision to sign Perri's name on the consent form and then backdate it to February 2, 1993 is a major issue since Ms. Perri was not the one to perform the procedure. Ms. Perri would be held responsible for the informed consent form not being signed when in reality it was Dr. Witt who performed the procedure without an informed consent form signed. Ms. Perri only looked over Mathias's chart. She would be the one strictly involved in the court case when she would be the one also responsible for also providing Mathias with the risks associated with the tubal ligation. If anything went wrong with the tubal ligation, Ms. Perri would be the one facing the consequences associated with the procedure since the informed consent was not signed. This is unfair to Ms. Perri since Mathias wasn't her patient and she didn't perform the tubal ligation. Dr. Witt should have been the one to sign the informed consent if they were going to backdate it to February 2, 1993 since he was the one who performed the procedure.
