Question

You the nurse is caring for a 54-year-old male client that was admitted to hospital for surgery.

The client orders include: NPO, peripheral intravenous (IV) normal saline (NS) 1000mls over 8 hours to be started at 0800 this morning then reassessed, routine blood work (CBC, electrolytes).

1. What type of solution is normal NS? Why is this solution prescribed for this client? How much IV fluid will this client receive hourly? And how often will you check the infusion rate?

2. The client informs you that he has never had an IV before. Based on this you identify a nursing diagnosis of: Knowledge deficit related to inexperience with IV as evidence by clients statement "I have never had an IV before"

What health teaching will you provide to this client about having an IV?

3. The nurse has initiated the IV NS infusion. The client calls you and informs you that his IV has stopped running. What factors influence the flow of the IV solution?

Your shift ends at 1530. You check the IV bag at shift change and note that there is about 50mls left.

4. What would be your intervention? What data would you document in relation to the client having an IV? What will you include in your transfer of care?

5. The physician order for this client's IV was changed from 125ml/hr to 150ml/hr at 1600 when reassed. At 1900hrs evening the nurse enters the clients room to complete an assessment. The nurse notes that the IV NS bag has 100ml TBA.

The assessment of the client includes: crackles bilaterally noted with chest auscultation, he is sitting in a tripod position, the client states he is short of breath, and has a full bounding pulse.

6. What do you think is happening? What nursing interventions would you implement and what collaborative care would you anticipate?

7. What other complications would you assess a client with an IV for? What would your nursing interventions be for each complication?
