Question

Patient was climbing a ladder and lost his balance and fell on his right elbow. He has severe pain and is unable to flex or change position of the elbow. He had no LOC. His family/social and past history are noncontributory. X-ray reveals a fracture of the humeral shaft and a fracture of the distal radius/ulna as well. CBC was normal. Patient in acute pain, VS normal. Right upper extremity shows obvious fracture. He is also tender across the distal radius/ulnar. An IV was placed, and a total of 18 mg of IVP Morphine were administered. He was also treated with 30 mg of IVP Toradol. As we are unable to treat this severe injury at this facility, he was given 75 mg Demerol IVP and 50 mg Vistaril IVP for transfer to another facility for treatment.
