Question

Close incident report about Patient came in with A. fib RVR, EKG HR 150. Patient ordered for tele. Patient sent up by ED without tele. Unclear as to why this occurred, discussed with night nurse that no report was given to him. Discussed with AM charge as well. Patient requires telemetry as this was one of the indications for admission and it is unsafe for patients to be brought up from ED unless telemetry box is obtained. Per nursing note nurse able to pick up the patient from the ED. No tele box and cont pulse ox available in the telemetry at the time.
