Question

Rewrite this SN made a visit to patient home, greeted by patient at the door. SN completed assessment on patient, vitals WNL, patient denied pain. No change of condition noted on patient. SN taught patient/PCG about home safety and fall precaution. SN assessed home for safety and falls. SN instructed patient/PCG to keep the environment well lit. Install and use handrails on stairs inside and outside the home. Wear shoes or slippers with nonskid soles both inside and outside the house. Keep pathways clutter free. Patient was instructed home safety measures such as using/placing his assistive devices within easy reach using long-handled tongs rather than climbing on a chair to reach objects. Install and use grab bars in the shower and near the toilet. Patient advised to use assistive device (walker) to ambulate.
