Past psychiatric history: Three past hospitalizations. Last hospitalization 3 years ago when BC overdosed on prescription medication. Medication trials: None Psychotherapy or Previous Psychiatric Diagnosis: Major depressive disorder (MDD), Generalized anxiety disorder (GAD). Family Psychiatric/Substance Use History: Brother; MDD, ETOH. Sister: MDD. Mother: Dementia. Psychosocial History: BC was raised by his mother; he lives with his girlfriend. He does not work. He has no current DUIs or legal issues. Substance Current Use: Current everyday cigarette smoker- 1 pack per day Medical History: Seizures, Congestive heart failure, COPD-Bronchitis/Emphysema.

Current Medications: Xanax 1mg oral tablet twice daily. Started: 5/15/ 2021, Effexor XR 75mg oral Capsule, extended release. Take 3 Capsules (225mg total dose) daily. Vraylar (Cariprazine) 1.5mg oral Capsule every day. Started: 11/10/2020; Zyprexa 2.5 mg oral tablet every night at bedtime. Started: 07/09/2020. BC reports not taking Zyprexa orally for the last two nights.
