Question

The patient has been in your intensive care unit twice in the past with acute pancreatitis associated with excessive alcohol consumption. During his stays in the hospital he has been noncompliant, demanding, and hostile, often becoming sarcastic and insulting to the nursing staff. He has been made aware that the continued excessive use of alcohol will present a serious hazard to his health and his life. When you receive the patient report before beginning your shift you are informed that he is again a patient, has ceased experiencing delirium tremens only within the last hour, and has struck a nurse, leaving bruises. Ten minutes into your shift he presses his call light and with profanity angrily demands pain medication. How do you respond?
