A patient came into the pain clinic for management of chronic neck and shoulder pain after a motor vehicle crash one year ago. The vehicle she was driving struck a bus on a country road. The pain extended down into her left hand, and the patient had difficulty lifting or moving anything with that hand. She also reported inability to sleep well due to pain. Her attending physician requested that a pain specialist take over the care for her pain. The pain specialist performed a medically appropriate history and physical exam and medical decision making of low complexity. He and the patient discussed the injection of Marcaine and steroids into the cervical plexus for relief. The patient agreed to this plan, ad after consents were signed, the injection was performed. The patient noted approximately 40 percent relief in pain almost immediately. The doctor requested that the patient come back in one week and again in two weeks for another injection.
