1. Primary Care Medical (PCM) Provider: Outpatient setting. The PCM is normally the first setting a patient encounters, where the physician evaluates the patient, performs different tests, and/or refers the patient to a specialist. In the case of Mr. Tod, he has been complaining of shortness of breath with physical activity. Upon further testing and lab work, the PCM provider referred Mr. Todd to the cardiologist for further evaluation.

2. Cardiologist: Outpatient setting. The cardiologist discussed Mr. Todd's findings and stated he would need open heart surgery as soon as possible due to clogged arteries, which puts him at a very high risk of a heart attack at any moment.

3. Acute Care/Hospital: Inpatient setting. In this setting, Mr. Todd undergoes an open heath surgery and receives initial nursing, physical, and occupational therapy care. The hospital communicates and sets up the next level of care with Mr. Todd's facility of choice.

4. Skilled Nursing Facility: Short-term care setting. This is the next level of care, where Mr. Todd will most likely spend four to eight weeks until he recovers his strength and re-learns how to engage in these daily tasks as independently as possible and safely. He will also receive nursing care and follow-up appointments with the cardiologist.

5. Home Health Care: Home setting. Mr. Todd will be discharged to his home if he is deemed capable of returning home safely with family support. Alternatively, Mr. Todd could choose to stay in long-term care.

6. Community Reintegration: Community setting. This setting allows for Mr. Todd to be re-integrated into the community and the things he enjoys doing, such as grocery shopping, meeting for Bingo with his friends, and engaging in community programs to promote good health.
