Question

Organizational models that attempt to integrate patient care can have different structures and approaches. One example is the Accountable Care Organization (ACO). This model brings everyone together to deliver the best quality of care possible, while remaining cost-effective. The main focus is to improve patient outcome and keep costs under control. Another example is the Patient Centered Medical Home (PCMH). This is when the doctor and staff coordinate/ manage patients care across different healthcare settings. Patient centered care is very effective and emphasizes the importance of coordinated and personalized care. Lastly, a third example I am familiar with is Health Information Exchanges (HIE's). These exchanges allow providers to share information and patient data among each other, while keeping the information secure. HIE's improve coordination among staff and allow us to provide the best quality of care possible. Each of these models are focused on improving patient outcomes and delivery of care. Each health system/ community requires different needs, which calls for different types of organizational models that attempt to integrate patient care at all times.
