Sometimes, as an Oncology Data Specialist, when abstracting information into a case, there will be information that is conflicting, ambiguous, and/or incomplete. Some examples are: a suspensed case has August 22, 1996 listed as the birthdate and the facilities medical record states August 22, 1969 for that same patient a surgical pathology reports final diagnosis may list 15 lymph nodes removed during surgery however the synoptic of the same pathology report may state 13 lymph nodes removed Oncology Data Specialists track alcohol and tobacco use during case abstraction; often, tobacco is listed as "non-smoker" in a physician's History and Physical Exam and "everyday smoker" by a different physician in their H & P What would you do in these situations? How do you know which documentation to follow? What procedures would you follow to resolve the documentation discrepancies?
