Question

Patient has been visible on the common areas of the unit this shift. He was mostly observed pacing the unit hallways while self dialoging. He was also very disorganized, restless, argumentative and oppositional to redirection. At times he appeared distant, preoccupied and responding to internal stimuli. He also was observed obsessively cleaning his room and using a tooth brush to clean his clothes in his bathroom sink. He had minimal to no interaction with his peers or staff except for needs. On approach he was confused, disoriented, at times hyperverbal with rambling, pressured and tangential speech. He did not attend any groups this morning or afternoon. No unsafe behavior noted or reported. Nursing staff will continue to monitor him throughout this shift for safety and needs.
