Anthropometric Data:

Weight in kg: 3, 5.2, 5.4, 5.6, 5.8

Length in cm: 50, 58, 62, 63, 64

Dietary History:

Peter's mother has gone back to work and transitioned him to bottle feeds using a standard baby formula. He continues to spit up frequently and seems "colicky." On average, he feeds 5 to 6 times per day and drinks 4 to 6 oz per feed (although spits up frequently ).
