Question

MDD adult, suicide risk, medical co-morbidity

LG is a 55 year old African American female you have been seeing for the last 5 years in the community mental health center. She lives with her daughter who has a hx of multiple drug substance use, in recovery, and 3 grandchildren. Lenore has described a history of depression since she was 25 years old. She has been hospitalized 3 times for suicide attempts, once for trying to hang herself, once for an overdose of trazadone, and once for walking into traffic. Her last suicide attempt was 2 years ago (attempted hanging). Over the years, she has been treated with 80 mg of Prozac (1 year), 200 mg of Zoloft for 6 months, and currently on combo of 400 mg of venlafaxine XR and bupropion XL 300 mg in am for the last 4 months. She can't remember other medications tried. She remains depressed but better, passively suicidal with no active plan but wishing "God would take me". Her mood is now 5/10. She still has anhedonia but better since you added the bupropion 4 months ago. She denies any drug or alcohol use.

She has hypertension and type 2 diabetes and is being treated with lisinopril and metformin. Her Hgb A1C has been 9 and her BP has been stabilized at 127/82, pulse of 72. Her glucose is usually elevated at 160 but the rest of the CBC, comp panel, thyroid profile is WNL. Her LDL is elevated at 150, total chol and triglycerides are WNL. Her BMI is 26. She is sleeping well but has gained 5 lbs in the last month. She denies any hx of hypomania/mania/psychosis.
